Job Summary

Baptist Health is seeking to add a Care Coordination Assistant within the Family Support Services department at Wolfson Children's Hospital. This is a full-time, day shift opportunity.

Under the direction of a licensed professional, the Care Coordination Assistant provides non-clinical and clerical support to the Pediatric Social Work and Care Coordination teams to help facilitate coordinated, patient-centered care. This role plays an important part in supporting smooth care transitions for patients and families.

Care Coordination Assistant, Job Responsibilities:

  • Provide non-clinical and clerical support to the Care Coordination and Pediatric Social Work teams.

  • Assist with coordination of care and discharge planning activities.

  • Support patient and family care transitions across the continuum of care.

  • Communicate with internal departments and external agencies as directed.

  • Maintain accurate documentation and assist with data entry in the electronic medical record.

  • Coordinate meetings, referrals, and follow-up appointments as needed.

  • Ensure timely completion of assigned administrative tasks.

Care Coordination Assistant Experience:

  • 1-2 years Hospital Experience Required

Education & Credential Requirements:

  • High School Diploma required
  • Associate of Arts/Science preferred
  • Basic Life Support (BLS) Required
  • Certified Clinical Medical Assistant (CCMA) Preferred Or
  • Certified Medical Assistant (CMA) Preferred Or
  • Registered Medical Assistant (RMA) Preferred Or
  • Notary Public Preferred
  • National Certified Medical Assistant (NCMA) Preferred Or
  • Driver's License Preferred
